The Life Story Book Template was designed and developed by hospital workers to help patients maintain dignity and improve their care.  The Life Story Book is a document that records your own personal history for anyone who needs or wants it.

Anyone can create his or her own Life Story Book by printing this page and filling in the form.  The completed form can be used for family history, personal information, or it can be taken with a patient into hospital for use by carers and staff.  The information on the form can help carers provide more personalized care for those who cannot speak for themselves.
